Based on your insurance or payment type, please print and fill out the corresponding Additional Required Forms, then click the Patient Portal button and complete your registration prior to coming to your first appointment. Please bring the completed forms with you to your first appointment.

Patients With Medicare:

Patients With Private Insurance / Workers' Compensation / Self Pay:

Billing Responsibilities and Insurance Information

Our fees are reasonable and customary for the provision of hand therapy services in the Conejo Valley.

We bill all health insurance providers. We are in-network for all major insurance carriers. Please call our Westlake Village office at (805) 497-1700 if you would like to inquire about insurance companies that we are currently providers for. All insurance companies have different plans for reimbursement. It is your responsibility to verify your insurance coverage and benefits.

Meridian Hand Therapy will treat any patient under the Worker's Compensation system who has been authorized by their carrier/adjuster. We will work with your physician to obtain this authorization.
